What is the Watson Headache Approach?

The Watson Headache Approach is a specialised physiotherapy assessment and treatment method focused on the upper cervical spine (C1–C3) — the top three joints of the neck.

Research by Watson and colleagues has demonstrated that dysfunction at these joints can sensitise the trigeminocervical nucleus — the brain's headache processing centre — contributing to or causing a wide range of headache types.

The approach involves:

  1. Precise manual examination of the upper cervical joints

  2. Sustained pressure applied to the affected segment

  3. Observation of whether this reproduces and then reduces the patient's headache — confirming the cervical contribution

  4. A structured course of targeted manual therapy to address the dysfunction

What Does the Assessment Involve?

Your Watson Headache assessment is not a standard neck examination. Your physiotherapist will:

  • Take a detailed headache history (frequency, location, triggers, duration, associated symptoms)

  • Perform a postural and cervical spine screening

  • Apply sustained manual pressure to specific upper cervical segments (C1, C2, C3)

  • Note whether the pressure reproduces your familiar headache — and whether it eases as pressure is sustained

This process both confirms the cervical contribution to your headaches and identifies exactly which levels need treatment.
